Recognizing the fact that a fic can explore relationship issues between two characters without having to go to that *sex* place is the first step to becoming a great writer. Kudos to you for getting that. It's not always necessary to take it there and I like that this bit doesn't. It just shows a man hurting and wondering when he became incapable of listening to those around him, it's a vivd piece.
So of course continuing it might be a fun excersize, but at the same time leaving it as is says something about Spike's struggle, the emphasis here is on the fact that it's not over, and that's important too. He's only just coming to grips with the fact that he can't just brush it all of as 'didn't have a soul', watching his past catch up to him will be exciting.
